The role involves management of patient/client/whānau with chronic conditions (lung disease, diabetes, heart disease, cancer support services, and respiratory services). We currently have 2 full time rewarding positions available. The focus is holistic patient-centered whānau ora healthcare services ensuring whānau develop the knowledge, and receive the full range of primary and secondary care support necessary to manage their long-term conditions.
Required Skills, Experience & Qualifications
· Registered Nurse with a current Annual Practicing Certificate
A minimum 2 years post-registration experience working with whanau in the community with long-term conditions would be an advantage
High level of data entry, reporting and administrative skill are required as is good communication skills
An understanding of Māori culture and protocols would be an advantage
Experience working with Māori communities would also be advantageous, particularly if you have an established understanding of, and network within Taranaki.
As this is a mobile position a full driver’s license is essential.
You must be eligible to work in New Zealand to be considered for this position.
