The role:

This role plays a critical part in delivering the health intervention component of the Tiakina te Tangata programme. Long Term Conditions Nurses will be responsible for engaging with participants and whānau to deliver interventions, as well successfully collaborating with primary and secondary care service providers to reach improved patient health and social outcomes. 

This is a community-based role as part of our mobile team where you will be visiting whanau in their own homes to provide clinical assessments and recommended pathways. Although often supported by Kaimanaaki, this is a highly autonomous role where no 2 days are the same. The role varies but you will generally spend your week in community and working closing with our clinics across South Waikato and at times may be needed to support in the wider Waikato region. A phone, laptop will be provided to the successful candidate.

About you:

We are looking for an experienced and confident nurse who has Recent Primary Care and/or Long-Term Condition experience and is comfortable working in a community setting. No 2 days are the same so you will be happy to work autonomously in a changing environment while being supported by the central NHC hub. 

You will have strong communication skills and be able to relate to people from all cultures and walks of life. Ideally you will have experience with Māori models of care or exposure working with Diabetes or other long-term conditions.

Desired experience:

• A Registered Nurse with a current Annual Practising Certificate 

• Basic life support with anaphylaxis 

• Full New Zealand driver’s license

• Be able to work in partnership with other health professionals to support patients with long term conditions. 

• Excellent organizational and time-management skills

• Outstanding communication and interpersonal abilities

• Ability to work autonomously and adapt in a rapidly changing environment

• Ability to relate to people from different backgrounds, in particular building rapport and strong professional relationships with Māori

• Existing knowledge of and willingness to grow and extend knowledge of te ao Māori and te reo Māori.
